Patellar tendon pain in cycling often feels very specific, which is why riders are tempted to reduce it to one simple cause. In practice, that is rarely enough. Load at the front of the knee is usually shaped by saddle height, cleat position, foot mechanics and the way you are taking load through repeated pedal strokes.

How cycling patellar tendon pain develops

The patellar tendon connects the kneecap to the shin and carries the quadriceps force that turns the pedal. It is loaded on every downstroke, so the question is never whether it works hard, but whether the setup is asking it to work hard in an unfavourable position, thousands of times per ride.

Two mechanisms come up most often. In the first, the knee is working through too much bend at the top of the stroke: the saddle is low, or too far forward, and the tendon takes its highest force at the joint angle where it is least well supported. In the second, the force itself is too high for the tendon’s current tolerance because the gearing is heavy and the cadence low, so each stroke is a small strength effort rather than a smooth one.

Common causes of cycling patellar tendon pain

Patellar tendon symptoms rarely come from one number alone. The key question is how the load is being built at the front of the knee and whether the setup is increasing that load unnecessarily.

  • saddle height is too low, so the knee works through excessive bend under force
  • saddle is too far forward and shifts load onto the front of the knee
  • cleat position and foot mechanics are loading the knee badly
  • too much pressure is occurring in the front of the pedal stroke
  • cadence is habitually low and the gearing habitually heavy
  • lack of stability through the hip and foot increases knee stress
  • training load and bike setup are not working together well

Two typical patterns: heavy gears and a slow build over distance

Some riders feel the tendon on hard efforts, climbing, or when starting off in too big a gear. That is a force problem: a lot of quadriceps load arrives at once, at a deep knee angle. Others feel nothing sharp at all, and instead notice a dull front-knee ache that builds over long rides and is worst the morning after.

Both point at the front of the knee but need a different emphasis in the fit. For the force pattern I look first at saddle position and at what your gearing and cadence are actually doing. For the slow-build pattern I weigh total position, foot stability and how quickly your training volume has changed.

What you can check yourself before you book

A few observations help me place the pattern faster. You do not need to fix any of this, just notice it on your next ride:

  • Is the pain just below the kneecap, or on the kneecap itself, or to one side?
  • Does it come on during hard efforts and climbs, or only after long steady riding?
  • Is it worse going down stairs, or after sitting for a long time?
  • What cadence do you actually ride at on the flat, and does the pain change when you spin an easier gear?
  • Have saddle height or position, cleats, shoes or training volume changed recently?

None of this replaces an examination, but it usually already shows whether saddle position or load management is the main story.

What I check and change during a patellar tendon bike fit

I first assess movement and control off the bike and then analyse the position under load. I want to see which factors are actually driving stress into the front of the knee rather than only reacting to the painful spot.

Typical levers are saddle height where the knee is working through too much bend, saddle fore-and-aft to move load away from the front of the joint, and cleat position and foot support so the knee tracks without fighting the foot. Cadence and gearing usually get discussed too, because they change tendon force more quickly than any component does. No change goes in on its own, and each one is tested straight back under load.

The goal is a position that reduces stress on the patellar tendon while keeping the rest of the system supported. You leave with clear target numbers, so the change still makes sense after the session.

Can you keep cycling with patellar tendon pain?

This is the question riders actually ask, and the honest answer is usually a qualified yes. Tendons generally respond better to reduced, controlled load than to complete rest, so riding easier gears at a higher cadence, on flatter routes, often stays sensible while the setup is corrected.

The useful test is how the pain behaves. Low-level discomfort that stays flat during the ride and settles within a day is usually acceptable. Pain that climbs as the ride goes on, wakes you at night, or shows up walking down stairs is a signal to stop riding through it and get it assessed. Riding at your normal intensity on an unchanged setup is what tends to keep the irritation alive.

When a bike fit helps and when medical assessment still matters

A bike fit is most useful when the bike or riding position is part of the load causing the symptom. It does not replace medical assessment for acute knee pain, swelling, giving way, locking, or pain that is not clearly linked to riding. If the tendon is already badly irritated, the sensible route is often both: have the load assessed medically or by a physiotherapist, and change the setup in parallel so riding does not keep re-creating the irritation.

The useful question is whether the setup is repeatedly asking too much from the front of the knee on every ride.
